Always rotated 4 tires and leaving spare as is. Rotating spare into the equation seems like a pain for very little gain. Rav4 hybrid seems to wear tires much more evenly than our previous FWD and AWD vehicles. Probably has something to do with the more balanced power and braking characteristics of the front and rear traction motors.
